Collagen shots for joints: pain relief or placebo?

For athletes with activity-related joint pain and individuals with mild to moderate knee osteoarthritis, clinical studies show measurable pain reduction after 12 to 24 weeks of collagen supplementation. The key study by Clark et al. (2008) in 147 young athletes showed significant pain reduction compared to placebo with 10g per day. Later meta-analyses confirm similar effects on WOMAC pain scores in osteoarthritis, comparable to glucosamine. How does collagen work on cartilage?

Articular cartilage consists of approximately 70 percent water and 30 percent collagen matrix (mainly type II). With age, cartilage density and water content decrease, leading to stiffness and pain. Collagen peptides are transported via your bloodstream to chondrocytes (cartilage cells). Two things happen there: the peptides provide building blocks, and specific bioactive peptides signal the cells to produce more of their own cartilage matrix.

The Clark study (2008): the first major RCT

147 young athletes with activity-related joint pain, 24 weeks, 10g collagen peptides per day versus placebo. Result: significantly lower pain scores on multiple parameters (at rest, during activity, after activity). The greatest effects were seen in knee and shoulder complaints.

Does it also work for osteoarthritis?

A meta-analysis in Current Medical Research & Opinion (2018) looked at 5 RCTs with patients with mild to moderate osteoarthritis. Result: significant improvements in WOMAC pain scores after 12 weeks, with effect sizes comparable to glucosamine or chondroitin.

Important: for severe osteoarthritis (grade 3 or 4), collagen is supportive, not a replacement treatment.

How much collagen for joints?
